Zawodowy zabójca Al Simmons zatrudniony jest przez tajną agencję rządu amerykańskiego o nazwie A-6. Chce porzucić to zajęcie, by poświęcić więcej czasu rodzinie. W trakcie sekretnej misji w Korei Północnej, której celem jest zniszczenie fabryki broni biologicznej, Al zostaje zamordowany przez swojego szefa. Po pięcioletnim pobycie w piekle powraca jako Spawn, humanoid o bardzo oszpeconej twarzy, ale wyposażony w rozmaite zdolności bojowe.
